Updated Payment Method

Updated payment methods are expected to be on the Patient Portal, before the patient is seen by the provider. Any complaints or requests for refunds will need to be addressed with office manager. Refunds may be issued on a case by case basis where warranted.

Accepted forms of payment

We accept cash and checks, but patient must have an active debit or credit card on the Patient Portal to gain eligibility to schedule a new appointment.

Appointment Deposit and Card-on-File Policy

At Cox Family Medicine, we value your time and strive to provide high-quality care while maintaining availability for all patients. In order to reduce missed appointments and last-minute cancellations, we require a refundable appointment deposit and a current credit or debit card on file for all new patients when scheduling.

How It Works:

  • A $50 refundable deposit will be charged to your card at the time your appointment is scheduled.
  • If you attend your appointment, the deposit will be applied toward your copay, deductible, or visit balance.
  • If you cancel your appointment with at least 24 hours’ notice, the deposit will be fully refunded.
  • If you do not show up or cancel within 24 hours of your scheduled time, the deposit will be forfeited as a no-show fee.

Why We Require a Card on File:
Keeping a secure card on file allows us to simplify billing and streamline patient check-ins. Your card information is stored securely through our HIPAA-compliant payment processor, and it is only used for:

  • The refundable appointment deposit
  • Copays, deductibles, or balances that you authorize
  • Applicable no-show or late cancellation fees as described above
